The Arizona Trauma and Acute Care Consortium (AZTrACC) will be sponsoring the Third Annual Southwest Trauma & Acute Care Symposium on November 3-4, 2011. This will be held at the beautiful Scottsdale Plaza Resort in Scottsdale Arizona.

Exhibit area will be in a beautiful setting adjacent to the main assembly. All meals and breaks will be held in this area to facilitate networking. WiFi is available throughout the hotel.

We anticipate a large attendance representing a national audience of trauma surgeons, orthopedic surgeons, other physicians, physician extenders, nurses, EMS personnel, and other healthcare professionals who have an interest in trauma and critical care. Guest faculty have been confirmed are nationally and internationally recognized.

In order to host quality educational events such as this national symposium, we depend on support from your organization. Exhibit space is available for an Early Bird Fee of $1,250 until October 1, 2011. The fee will increase to the regular rate of $1,500 after October 1, 2011. This fee includes a 6 foot, draped table, two chairs and electrical outlets in an 8 foot booth area. There are also opportunities for additional sponsorships as listed on this form.

Based on last year’s positive feedback, we will have some of the same interactive opportunities in an effort to encourage more networking between our attendees, faculty, and vendor representatives. We hope that you will be interested in participating as an exhibitor. You may complete and return the Exhibitor Contract and Sponsorship Agreement form or fax it to 602-839-6229. Please also read the Exhibit Rules and Regulations. Checks or Educational Grants may be made payable to AZTrACC, Tax ID # is 26-0380437, and mailed to the address located on the form.
